To Each Attorney/Self-Represented Party:

Please provide written answers to each of the following questions:

1)     State your name, address, e-mail address, facsimile, and telephone numbers.

2)     If you are an attorney, state the name and address of each of your clients in this case.

3)     If this case is not filed with a court, state the name and address of each of the parties to the dispute in this case.

4)     If this case is filed with a court, identify: (a) the court; (b) the full case name; (c) the case number; (d) each plaintiff, defendant, cross-complainant, cross-defendant, intervenor, interpleader, and lien claimant; and (e) any trial date.

5)     State the name, mail address, e-mail address, facsimile, and telephone numbers of each attorney and self-represented party in this case and, for each attorney, identify each of that attorney’s clients in this case.

6)     Identify any attorneys who previously represented any party in this case.

7)     State the name and address of any persons who, while not a party to this case, might have a significant interest in its outcome.

8)     Please provide a concise description of this case (including all claims made, remedies sought, defenses asserted, amounts/interests at issue, and the basic “who, what, when, and where” of the incident(s) giving rise to this case).

9)     State the names and addresses of any potential witnesses in this case.

10)   How many days do you estimate the arbitration hearing should take?

11)   On what dates are you and your client(s) available to attend a hearing?

12)   Do you want the arbitrator to issue a tentative opinion first?

13)   Identify any pre-hearing matters the arbitrator may need to resolve?

14)   Describe each relationship, contact, and/or dealing, if any, which you and/or your client(s) have at any time had with Haggerty Arbitrations and/or John K. Haggerty.

Please send your completed responses to Haggerty Arbitrations and copies of those responses to each of the other attorneys/self-represented parties in the case.
